Treatment Agreement
General Terms and Conditions
All work we perform in connection with examining or treating your animal is subject to the General Terms and Conditions of the Royal Dutch Society for Veterinary Medicine in force from time to time.
Costs
On request we can give you an indication of the costs of treatment and/or examination. It remains an indication, costs may vary. If possible we will contact you. In some cases, in view of the welfare or health of your animal, it is not possible to contact you to agree on the additional costs first.
You must pay the cost of the consultation, examination or treatment and the necessary medication afterwards in cash or by debit card transaction, unless we make other arrangements with you about this in advance. The actions taken are processed in our administration system.
Third Party
If someone other than yourself brings your (domestic) animal to our practice, we assume that this is done at your request. We will then also examine and/or treat your animal applying the agreements and conditions described in this letter. The person who presents the animal to us is responsible for paying the costs incurred (so this is not necessarily the owner), unless otherwise discussed.
Privacy
Of course we treat your personal data strictly confidential, but it may sometimes be necessary that we share your personal data with third parties who are involved in the research or treatment of your (pet) animal. This may include the insurance company of your pet, a specialist or laboratories. With personal data we mean your name and address, email address, date of birth and the (medical) data of your (pet) animal, including the possible chip number. We keep your personal data for 5 years after the last treatment or as much longer as necessary to protect our legal status. The Privacy Regulations of our practice can be found here.
